There is no point in having random categories which do not match your interests at all. This is my personal tip. Once you have chosen categories, you have to keep adding related words to each category in your list. I usually recommend focusing more on adjectives than nouns. At times, I would even divide the list of categories into adjectives and nouns. Each category would have a column of noun and adjectives. What level of fluency is expected from you?

As mentioned earlier, at the TOEFL exam your skills are assessed based on how well you understand the words in your vocabulary list. Keep in mind that the TOEFL test will require you to read academic papers and texts in the reading section for vocabulary-related questions. Hence, it is mandatory to read texts, books, non-fiction, newspapers and keep track of the words that are being used in these texts from the vocabulary lists.

You have to be familiar with how the words are used in different texts. Just note down that word and the sentence and check their meanings after you read the whole text. Even before you pick up the dictionary try to guess the meaning of the word. Then check whether you were correct. You are more likely to remember the word and the usage if you do this simple exercise.

It is also crucial to understand the meaning of the word and in what situations you can use it. Furthermore, I recommend watching TED talks with subtitles and a transcript on the side. This will help you understand how certain words are used in a sentence or a particular phrase. The talks on the TED official website are usually accompanied by a transcript, which you can look at as you listen to the conversation.
